Seeking BE/BC Palliative Care physician to provide palliative consults, provide hospice homecare visits, evaluate for hospice certification/recertification of terminal prognosis and quality of plans of care, ensure accurate documentation to support reimbursement for services provided and participate in on-call rotation as needed.
